New Freedom Health cares about your privacy. We will not sell or share your Personal Information without your permission.

This Privacy Policy outlines the types of Personal Information we may obtain from you and how we use and protect it. This Policy applies to our collection of infor-mation through our website, forms you fill out as our client, written and email correspondence you have with us, and telephone calls between you and members of our staff.

Your Consent to This Privacy Policy

When you provide us with Personal Information, you are authorizing us to use that information in accordance with the terms of this Privacy Policy which includes your right to limit our use of your information. If you do not agree with any of the terms of this Policy, please do not provide us with your Personal Information.

To provide services to you, we will ask you for Personal Information. As used in this Privacy Policy, “Personal Information” means information that identifies you. This includes:

  • Contact Information – Your name, address, telephone number, email address, and emergency contacts.
  • Financial Information – Banking, credit card, and insurance information.
  • Health Information – Health history, current health status, medications, the names and contact information of your care team members, i.e., doctors, therapists, and caregivers, etc.

How We Use Your Information

We use your Personal Information to provide services to you. It enables us to:

  • Understand your needs.
  • Communicate with you and those you authorize us to.
  • Receive payment for the services we provide to you.
  • Coordinate your therapy with other members of your care team.

We will provide your Personal Information to third parties only for the express purpose fulfilling an order you place with us. Finally, we will release your Personal Information when required to do so by law.

You have the right to decline to give us any of the Personal Information we ask for. However, doing so may limit the services we can provide for you, or make you ineligible for therapy due to safety or other concerns.

Security of Your Personal Information

We are fully compliant with the Healthcare Information Portability and Account-ability Act (HIPAA), the federal law that protects your medical records and other health-related information (your Personal Information.) We employ multi-layered security protocols to keep your information safe. However, because it is not technically possible to defend against unknown cyber threats, we cannot guarantee the security of your Personal Information against data breaches.

Changes in Our Policy

We reserve the right to implement changes to this Privacy Policy at any time. If you are enrolled in therapy when a change is made, we will notify you by email.
